| Abstract: | We present a novel approach to cognitive assessment that harnesses immersive virtual reality and advanced artificial intelligence. Our system integrates an adaptive AI agent, SmartSage, to facilitate engaging and personalized evaluations of cognitive function in real time. By embedding AI-driven guidance into a VR environment, the system encourages naturalistic sensorimotor interactions during task performance, offering a dynamic alternative to traditional, screen-based assessments. We detail the system's architecture, its interactive design based on embodied cognition principles, and preliminary user study outcomes that demonstrate enhanced engagement and feedback quality.
