Title: Synergistic Immunomodulatory and Regenerative Therapy for Superior Labral Repair
Abstract: Labral tears exhibit limited intrinsic healing and persistent inflammation, which often lead to tissue erosion following surgical repair. Our previous studies demonstrated that a platelet-derived growth factor (PDGF)-loaded bioadhesive enhances regenerative matrix deposition. However, while initial inflammation is essential for healing, chronic or excessive responses cause labrum erosion and limit therapeutic efficacy. To address this, we developed a dual-therapy strategy combining an injectable anti-inflammatory agent, cromolyn, with a PDGF-loaded bioadhesive. Using a rat torn labral model, we demonstrate that this combination treatment significantly reduces the inflammatory marker CD11b and cytokine IL-1β compared to the injury group. Concurrently, markers of tissue regeneration—including glycosaminoglycan (GAG) content and collagen type I expression—were substantially increased. Morphometric analysis confirmed that the combined treatment not only promotes regeneration but also significantly mitigates labral tissue erosion. Finally, gait analysis results confirm that this synergistic approach significantly improves functional recovery. These findings suggest that balancing immunomodulation with growth factor delivery provides a superior pathway for labral repair.
